Medical Exam Information

Recent events surrounding band students rehearsing in the hot Florida sun serve as a reminder that we cannot be too careful with regard to the health and well being of those participating in Marching Chiefs.

To participate in the Marching Chiefs ensemble-class, the following medical documentation is required of all participating students:

  1. Physical Examination (completed by a qualified health professional);
  2. Medical History (form completed by the student and brought to preseason training);
  3. Health Insurance Information (form completed by the student and brought to preseason training).

The following sections will detail each component listed above.

Physical Examination
All students accepted for enrollment in the Marching Chiefs course are required to complete a basic physical examination by a qualified physician prior to the first semester of participation. This complete basic examination is required on a one-time only basis upon first enrollment in the Marching Chiefs course. (In future seasons, returning students will complete a medical update questionnaire and blood pressure screening.) This requirement is based upon the same requirement established by the FSU Athletic Department for all FSU student athletes. The cost of the medical examination is the responsibility of each student.

Students have the option of completing this examination prior to arrival on campus (by your family’s regular physician) or at the Thagard Student Health Center on the FSU campus. The decision whether to complete the exam now or later is entirely your choice. Some may elect to do this now and avoid long lines at the Thagard Student Health Center. Others may choose to wait until the audition procedure is complete. Still others may decide based on cost effectiveness. (Be advised that the medical exam at Thagard is approximately $80.00. To schedule an exam at Thagard call 850-644-2026. Keep in mind that you may have the examination performed at the Thagard Student Health Service or your regular health care provider.)

It is preferable for new students to complete the examination prior to the strenuous preseason training session. However, we understand that time and cost issues may make the choice of an examination at the Thagard Student Health Center more desirable. If you are attending summer school on the FSU campus, you should consider making an appointment at Thagard at your own convenience as soon as possible.

This form must be completed and signed by a qualified clinician. The deadline for completing the examination is the first scheduled performance (home or away football game). If you elect to take the exam prior to preseason training, bring the completed form with you to preseason training registration. If you elect to receive the exam during or after preseason training, return the completed form to the band office (HMU 101) after it has been signed by a clinician. Forms will also be collected at the rehearsal field prior to daily practice.

Important: There is a place for you to sign indicating the release of the information to the FSU band office.

Medical History
This four-page document provides information about your health that is used by the Marching Chiefs staff in case of an emergency.

You are required to bring this form with you to Preseason Training in August.

If you elect to have your physical examination taken before preseason training, fill out the medical history form and take it with you when you see your family physician for your physical examination.

Insurance Information
This one-page document provides information about your health insurance that is used by the Marching Chiefs staff in case of an emergency. Having this information available will reduce the chances of delay in providing care during an emergency situation.

You are required to bring this form with you to Preseason Training in August.

Confidentiality
All of the medical and insurance information required for participation in Marching Chiefs is for use by FSU College of Music personnel only. All information will be kept strictly confidential. Every effort will be made to protect your privacy, but we must have this information in case of an emergency. Thanks for understanding.
